We conduct both prospective clinical trials and molecular epidemiological studies aiming to individualise breast cancer care, with focus on hormone-sensitive breast cancer. We collaborate with several strong research groups, clinical networks, and patient organisations.

Our Research

The long-term goal of Marike Gabrielson’s research group is to reduce recurrence and mortality in breast cancer by therapeutic prevention and personalised adjuvant therapy. This is done by focusing on two essential areas: identifying who will most benefit from preventive therapeutic measures, and by tailoring adjuvantcare of patients with breast cancer. 

Ongoing projects in the group include among others: 

  • The POWER trial; a randomised clinical trial to test individualised adjuvant therapy with tamoxifen for patients with breast cancer
  • Understanding the mechanisms of action of endocrine cancer therapy in preventing breast cancer, and identifying biomarkers for tailoring individualised preventive and adjuvant therapy
  • Identifying predictors of therapy discontinuation and therapeutic effect
  • Deciphering the molecular epidemiology of breast cancer risk, and identifying biomarkers of risk within the breast tissue and by hormone and proteomic profiles
  • Better understanding of the biology of mammographic breast density as a risk factor for breast cancer

The POWER trial for individualised breast cancer therapy

In EU and the US, a woman is diagnosed with breast cancer every minute. Most patients are treated with hormonal therapies, with tamoxifen or aromatase inhibitors, during 5–10 years. Tamoxifen is the often therapy of choice for premenopausal women. Up to half of all tamoxifen patients discontinue their treatment early, mainly because they do not tolerate the side effects. The outcome? Tens of thousands of women do not get the full preventative treatment – and are at risk of recurrence or death. Despite this, there is no monitoring of symptoms during the treatment. All patients receive the same dose, independent of how they tolerate it – and what dose they need.

In the POWER (Personalised dose Optimisation With adjuvant tamoxifen therapy in breast cancER) trial we want to individualise endocrine breast cancer therapy with tamoxifen, depending on each patient’s individual needs and body. This way, we think that more patients will remain on their treatment for the recommended time, and more patients will receive a clinically relevant dose.

The POWER trial is a randomised clinical trial in which we test if an individual dosing of tamoxifen based on endoxifen concentrations and side effect burden decreases the discontinuation rate and prolongs time of therapy, compared to standard of care and the same dose for all. 

The POWER trial is a multi-centre randomised phase III clinical trial where 1,100 tamoxifen patients are followed for five years. The trial will be conducted at several leading hospitals in Scandinavia. 

The POWER trial is led by a steering group of top leading breast oncologists and tamoxifen experts in Scandinavia. Karolinska Institutet is sponsor of the POWER trial, and sponsor representatives are Marike Gabrielson and Jenny Bergqvist.
